Request our help with transportation to a medical appointment

If you'd like to request our help with transportation to a medical appointment, give us a call on 07856 469508. Please make sure to:

• call between 09:00 and 13:00 on weekdays only

• leave a message if we're not able to answer your call, clearly stating your name, address and the date, time and address for your medical appointment

• give at least 48 hours notice for all trips

• ensure you are resident within the catchment area of Holmes Chapel Health Centre (not sure? Check your address here)

One of our trip Coordinators will match your requested trip with one of our local volunteer drivers. The driver will call you to confirm details and arrange a time to collect you.

On the day, the driver will collect you from your house at the pre-arranged time and transport you to your medical appointment. For shorter appointments they will stay on site to transport you home following your appointment. For longer appointments they may leave the site and return to collect you once you are ready to come home. The driver will confirm this with you on the day.

We ask that you consider making a donation to cover the cost of fuel for your trip. Below is a full list of recommended donations, and your driver will also be able to confirm the recommended donation for your trip on the day.
